The Centers for Medicare and Medicaid Services (CMS) created the STAR Ratings program to help consumers evaluate and compare the quality of Medicare Advantage (MA) Plans. Monetary bonuses – as much as 3-5% of a plan’s total Medicare payments – are awarded to plans achieving ratings of 3 or higher on the 5-point scale. These bonuses can be the difference between a plan being a loss leader or a profit center.

LifePlans helps MA plans improve their STAR Ratings with programs that help members stay healthy, manage chronic conditions, improve their level of member satisfaction, reduce the number of member complaints, and improve their telephonic customer service delivery.

LifePlans provides:

  • In-person HRAs enable plans to successfully enroll individuals in specific medical management plans
  • Improved care transitions are achieved by providing members with tools and support that promote knowledge and self-management as they move from the hospital to the home
  • HRAs, fall prevention and care transitions all reduce hospital readmission rates and costs for complex cases
  • Care coordination ensures continuity of care across multiple settings to improve the quality of care provided to members
  • Education and training enhance the health status of program participants through improved symptom management and functional recovery strategies

Only nine MA plans received a five-star rating for 2012 out of 569 total plans according to Medicare. Learn how LifePlans can help you improve STAR Ratings for contacting:
