We are pleased to introduce LifePlans, Inc.* as a preferred provider of innovative telephonic and in-person clinical Health Risk Assessment (HRAs), Care Transitions and Fall Prevention programs for Medicare Advantage, Special Needs and commercial health plans serving adult and aging populations. Through deployment of a network comprised of more than 4,000 nurses, LifePlans delivers these services efficiently, accurately, and effectively to obtain the results that health plans desire – improved member management, high quality service, and prevention strategies designed to control and reduce avoidable medical expenses.

Key highlights of LifePlans service offerings include:

  1. Assessment Services
    • Telephonic Health Risk Assessments

    • Through application of LifePlans’ clinical classification algorithms, members are classified according to their level of risk for an adverse event and are flagged for care managers so that timely action can be taken to improve management of the member and reduce costs.
    • In-Person Nursing Assessments

    • LifePlans can rapidly deploy Registered Nurses with geriatric experience to conduct in-person assessments in members’ homes. Assessments focus on functional, cognitive, medical and home safety domains.
  2. Member Health Management and Wellness Programs
    • LifePlans was chosen by the Department of Health and Human Services to design and conduct the only national demonstration of a Falls Prevention Program. This highly successful program has reduced fall rates by 29% over the course of a year and has reduced fall risk factors. The implementation of a Fall Prevention Program is directly tied to a Health Plan’s STAR rating.
    • LifePlans has successfully deployed its clinical network to deliver a Care Transitions Program designed to reduce avoidable hospital readmissions. Using member coaching and education, medication reconciliation, the development of health goals, and physician follow-up, the program has reduced hospital readmission rates by 25% over a 90 day period.
